New Patient Information.

Thank you for choosing Sports Medicine South as your Orthopeadic and Sports Medicine provider. In preparation for your upcoming appointment, we have set up a guide below. Please call our offices ahead of time if you have any questions or concerns along the way.

What Should I Do Before My Appointment?

All new patients must complete our Patient Forms packet prior to their appointment. If you are unable to print and complete the forms prior to your visit, please arrive 30 minutes early to allow time for paper work. Our Patient Form packet contains 4 different documents:

  1. Patient Information Record,
  2. Patient Consent,
  3. Financial/Credit Policy, and
  4. Narcotic Drug Prescription Policy

All patients must also complete their Medical History Form online prior to the initial visit. You will fill out this form through a secure portal, which will transfer directly to our system at our offices.

What Do I Need to Bring With Me?

We require that you have the following with you for your initial visit:

  1. Driver’s License or Valid Photo ID
  2. Current and Eligible Insurance Card
  3. Your 4 Completed Patient Packet Forms
  4. Medical Referral (if required by your insurance)
Do I need a Referral?

Some insurance plans may require you to have a valid referral from your family doctor in order to be seen at our office. It is the sole responsibility of the patient to (1) find out if the insurance plan requires a referral and (2) obtain the referral from your family doctor’s office prior to your visit.

What Time Should I Arrive?

We ask that you arrive as close to your schedule appointment time as possible. In order to be fair to all our patients, we cannot accommodate early or late arrivals. If you are more than 15 minutes late for your appointment, we will have to reschedule your appointment for another time that day or in the future. If you need to cancel an appointment, please call our offices at least 24 hours in advance as a courtesy to other patients who want to get in.

How Much Will It Cost Me?

Every insurance policy is unique and coverage types change from patient to patient. We are required to collect any co-pays at the time of service as well as any outstanding deductible amounts. If you are unsure of your benefits under your policy, please call the customer service number on the back of your insurance card.
