At Sports Medicine South, we take pride in assisting you with all your needs from medical to administrative. In many cases, outside agencies may require paper work and certified records to further process claims. Please use the drop down menu below as a guide to fulfilling your requests:

Medical records include documentation of your medical care delivered within Sports Medicine South by Sports Medicine South medical professionals including your doctor, physician assistant, hand and/or physical therapist, MRI and radiology personnel.

You may request your medical records in person, by fax, e-mail, or by mail.

Please note that medical records are processed as they are received. All records will be reviewed by the treating physician prior to being released – please allow time for processing. Some requests may incur a fee, please contact our Medical Records desk for further inquiries.

All disability forms requested have a 7 to 10 day waiting period for completion. This waiting period is to ensure that the accuracy of your current medical condition, as well as, the medical needs/requirements associated with your condition are transcribed in the most appropriate manner. Disability forms are information sensitive, and we do our best to complete them with the upmost precision so that you do not encounter any hold-ups when filing your forms with the appropriate entity. We will contact you upon full completion of your requested paperwork.

There is a $25 administration fee for each set of disability paper work requested

As a courtesy to our patients, we will fax your disability forms to the appropriate entity once complete. We keep all FAX CONFIRMATION sheets for our records; however, it is ultimately the patient’s responsibility to make sure that forms are received by the appropriate entity.

Prescription refill requests can be made by phone or online. Please note that refill requests are only for established patients who have been seen by a Sports Medicine South provider within the last three months.

It can take between 24 to 48 hours to process requests. Please anticipate your Rx needs to ensure no interruptions in your medication therapy. Requests are taken during normal office hours, 8:30am to 5:30pm Monday through Friday. Any requests made after hours, or on the weekends, will not be taken until the next business day.

All the following information is REQUIRED in your voicemail to Rx Refill line. If all information is not clearly provided, the Rx Refill Request will not be processed.

Please complete and submit the form below. Electronic Rx requests will be sent directly to our Medical Desk to be processed. If able, we will call in your Rx to your chosen pharmacy.

Any requests made for Schedule II, NARCOTICS are required by law to be picked up at our office in person
